Assists with all social work functions as described in Social Services Policy Manual.
Provides crisis interventions, brief interventions for bereavement, mental health counseling, and substance use disorder counseling.
Remain up to date and compliant with all state and federal mandates on mandatory reporting of abuse and neglect.
Provides case management, assessments, and completes infant safe plan of care for prenatal patients who have a substance use disorder or psychosocial need.
Performs comprehensive psychosocial assessment of patients;
Conduct substance use disorder assessments, that are gender-responsive and trauma-informed, utilizing ASAM criteria to determine the appropriate level of care.
Responds to referrals for assessment and brief intervention for both inpatient and outpatient in a timely manner.
Interacts professionally with patient and their families, and involves patients and families in discharge planning process.
Assist the hospital and families in applying for guardianship over incapacitated individuals.
Makes appropriate referrals and arrangements based on approved discharge plan by physician and patient.
Serves as social work consultant to other members of Health Care Team, physicians’ offices, outside agencies and community residents.
Advocates for patient and family needs in different settings: inpatient, outpatient, home, and in the community
Develops and maintains resource materials for use by hospital staff for the purpose of assisting patients and families.
Plans and implements appropriate in-services, education and trainings for hospital personnel.
Participates on hospital committees as requested by Director.
Develop effective working relationships with all community providers.
Coordinate and assure effective and timely delivery of all program services.
Educating hospital staff on patient psychosocial issues.
Complies with hospital and departmental policies, procedures, regulations and standards as related to job.
Demonstrates hospitals stated mission and values of teamwork, respect and communication with co-workers, patients, families and visitors.
Actively and appropriately participates in problem-solving identification and resolution, both intra- and inter-departmentally.
Meets department customer service standards and strives to enhance the level of customer service provided.
Active member of Discharge Planning Team.
Documents in medical records according to current hospital policy and procedure.
